YCN End of Life Care
Introduction
End of Life Care is care that helps all those with advanced, progressive incurable illness to live as well as possible until they die. It enables the needs of both patient and family to be identified and support to be given to help meet these needs.

End of life care includes management of symptoms, including pain and provision of psychological, social, spiritual and practical support.

End of life care refers to the care provided during the last phase of life and includes provision of bereavement support to family and carers after the patient has died. Identifying when an individual enters the 'end of life' phase can happen in a number of ways.

A set of core principles underpin the provision of high quality end of life care.

The Sub-regional Palliative and End of Life Care Group exists to support the achievement of the best possible outcomes for patients by ensuring access to a uniformly high quality of care and service provision across the Network area. The group includes representatives from across the YCN area and has an agreed 3 year work programme.
