 aims/objectives: The CNS and dietetic team developed a follow-up / diagnostic service.

The service allows patients to meet with their CNS and dietetic team throughout their diagnostic pathway. Follow-up post surgical treatment is also provided.

The team aim to meet the on going supportive care needs of patients along with a individual approach to follow-up care and survivorship.

The team are working with the Oncology team and 4th year medical students to look at the specific care needs of patients at different points of their post treatment pathway.

Individual care packages will be developed to meet the survivorship needs of individual patients. A move away from the "one size fits all" approach to follow-up care.

 cancer sites: Low to moderate risk breast cancer patients
 aims/objectives: A pilot study ,supported and run in collabouration with the YCN, Breast Cancer Care and the University of Huddersfield has run over two years, providing an alternative to hospital based, routine follow-up for low to moderate risk breast cancer patients.

The study providing a taught course aimed at empowering patients to better cope with the effect and the impact of cancer trearments as well as a better understanding of the signs of possible recurrence.This aimed to ensure that the patient was confident to self manage without hospital follow-up. Evaluations were collected after each taught session, timed questionniars over a three year period, and six monthly free script diaries provided the data for evaluating the service.

Early results indicated that providing an educational programme is at the least, no worse than hospital follow-up and has the potential to provide a better quality service , releasing clinic staff, time and funds for patients requiring greater clinical input. The study concluded that this service was both acceptable and evaluated very well as an alternative to hospital based routine follow-up and fit well within the suvivorship framework.

The project aims are to follow through a roll out in the network of the educational programme and open follow-up policy that has been provided routinely in the Calderdale and Huddersfield NHS Foundation Trust hospitals for the past year.

The two year project is to to provide assistance in each breast unit in the network to set up and establish their own educational programme suited to the needs of their population group and numbers.

 evaluation: The pilot study, which completed recruitment last year conrinues with data collection for another couple of years. The service has run at the CRH/HRI sites as routine practice for a year and is being evaluated with a questionnaire this year (2011).

It is anticipated that each new set up area will evaluate their practice annually also.
